Former child actor Blake Garrett<\/strong>\u2019s official explanation for death has been revealed.<\/p>\n

In accordance with documents from the Office of the Chief Medical Examiner in Tulsa, Oklahoma exclusively obtained by Us Weekly<\/em> on Monday, July 13, Garrett\u2019s probable explanation for death is listed as acute fentanyl toxicity. His death was ruled an accident.<\/p>\n

The summary report also stated that Garrett was residing in a sober living house in Tulsa, Oklahoma, on the time of his death.<\/p>\n

Garrett\u2019s mother, Carol, confirmed to TMZ<\/a> in February that the The best way to Eat Fried Worms<\/em> actor had died. He was 33. Carol told the outlet that Garrett visited an emergency room in Tulsa one week before his premature death. Garrett experienced \u201cintense pain\u201d before he was later diagnosed with shingles, per the outlet.<\/p>\n

Garrett also recalled a fun on set moment where he and the forged got to ride bicycles for a scene.<\/p>\n

\u201cThere was one scene where we were riding on a gravel road and got to slip to a stop. The blokes who could ride worked on that scene.
